  · must have been completed within one year (12 months) prior to enrollment date
  · EXCLUDE FROM SCHOOL UNTIL TURNED IN

  · up-to-date on a KY Immunization Certificate
  · must be verified by a school nurse
  · EXCLUDE FROM SCHOOL UNTIL VERIFICATION

Vision Exam                             
  · out-of-state transfers ONLY
  · must be on a KY form
  · documentatation provided within 30 days of enrollment
